What’s included: 

Standard membership

  • 1 comprehensive one-hour physical exam/well child exam annually

    • Lab reviews for diabetes, high cholesterol, vitamin D, blood counts, thyroid, kidney/liver function, stool testing (when applicable)

    • Review of recommended screenings and referrals if needed (colonoscopy, mammogram)

  • Standard office visits at least 30 minutes

  • Management of a wide variety of chronic conditions such as hypertension, ADHD, depression, diabetes, and high cholesterol with appropriate follow-up without a copay or coinsurance (typical copays range from $30-$50 and a simple follow up office visit can range from $125-$150)

  • Commercial Drivers License examination (if applicable)

  • Free in-office testing and procedures, including urinalysis, strep throat, urine pregnancy, blood glucose, and EKG (typically $30-$100)

  • Significantly discounted in-office testing and procedures (typically $100-$500) including but not limited to

    • Testing: influenza, computer-based ADHD testing

    • Procedures: Joint injections, ear wax removal, toenail removal, IUD insertion/removal, Nexplanon removal, and cryotherapy 

  • Access to discounted labs and in-office phlebotomy

  • Access to discounted generic medications

  • Access to workshops/seminars at discounted rates

  • Prompt scheduling for sick visits (24-72 hours)

  • Extended access via text/email/video calls

Enhanced membership

  • All services of standard membership

  • Preparation and review of annual personal health plan to be provided to patient

  • Integrative health coaching, using the 4M Method (movement, meals, mind, medicine), for management of diseases such as diabetes, hypertension, heart disease

    • Weekly to monthly data review (i.e. step counts, blood sugar or blood pressure logs, food logs)

    • Detailed nutritional guidance (as needed)

    • Detailed movement prescription (as needed)

    • Movement-focused visits (“walk and talk” visits)

  • Attend workshops/seminars at no cost
